Abstract: Introduction: Academic leagues are Brazilian student-led initiatives aimed at doing multiple extracurricular activities in a medical school. The leagues exist in almost all the universities of the country. Purpose: To characterize the activity profile of the academic leagues of a medical school. Method: Quali-quantitative cross-sectional descriptive study that collected data on academic leagues such as: year of foundation, number of members, number of teachers, types of selection and admission and activities performed. Results: Of the 52 active leagues, 27 provided data. The average number of students was 21.6 ± 10.4, ranging from 07 to a maximum of 48. Most respondents (77.8%) had only one teacher. The performed activities were mainly theoretical classes, discussion of clinical cases, practical activities, production of scientific articles, publications in social networks, solidarity activities and organization of their own events. Conclusions: Academic leagues are student-led initiatives with great potential to expand medical school actions using the existing structure. With their activities mapped and guided by management, academic leagues can be great catalysts for complementary activities and can contribute greatly to the training of new doctors.


Resumo: Introdução: Ligas acadêmicas são iniciativas de estudantes brasileiros criadas para realizar múltiplas atividades extracurriculares em uma escola médica. As ligas existem em quase todas as universidades do país. Objetivo: Caracterizar o perfil de atividades das ligas acadêmicas de uma escola médica. Método: Estudo descritivo transversal qualiquantitativo que coletou dados das ligas acadêmicas, como: ano de fundação, número de membros, número de professores, formas de seleção e ingresso e atividades realizadas. Resultados: Das 52 ligas ativas, 27 forneceram dados. O número médio de estudantes foi de 21,6 ± 10,4, variando entre 07 e 48. A maior parte das respondentes (77,8%) possui apenas um professor. As atividades realizadas são, majoritariamente, aulas teóricas, discussão de casos clínicos, atividades práticas, produção de artigos científicos, publicação em redes sociais, atividades de voluntariado e organização de eventos próprios. Conclusões: Ligas acadêmicas são iniciativas lideradas por estudantes com grande potencial de amplificar as ações da escola médica usando a estrutura já existente. Com suas atividades mapeadas e orientadas pela gestão da escola, as ligas acadêmicas podem ser grandes catalisadoras das atividades complementares e contribuir para a formação de novos médicos.

BACKGROUND: Hypertension is the most common medical condition during pregnancy. Hypertensive disorders are associated with an increased risk of adverse outcomes for both mother and fetus. This study examined the role played by personality and coping strategies in relation to blood pressure levels during pregnancy. The specific goal was to study whether coping strategies can mediate the effect of personality in pregnant women with hypertension. METHODS: A sample of 351 pregnant women was enlisted, encompassing 192 pregnancies complicated by hypertension. This is a cross-sectional study where personality traits were measured by the five-factor model and coping was evaluated by Jalowiec's coping inventory scale. RESULTS: Personality can partially predict systolic and diastolic blood pressure. Openness to experience trait is inversely correlated with systolic and diastolic blood pressure. Conversely, emotion-focused coping directly correlated with blood pressure levels. Systolic (ß = -0.14; p < 0.05) and diastolic (ß = -0.15; p < 0.05) blood pressure were also predicted by openness to experience. CONCLUSIONS: It is recommended to reinforce the development of coping strategies which focus more on the problem than on the emotion, avoiding detrimental effects of emotional coping in blood pressure levels during pregnancy.

BACKGROUND: Hypertensive disorders are important causes of maternal and perinatal morbidity and death. Considering the role of both physical and psychological factors in pregnancies complicated by hypertension, the aim of this study is to examine psychological factors and coping strategies in pregnancies complicated by hypertension. METHODS: Cross-sectional study. A sample of 552 pregnant women, 343 with pregnancies complicated by hypertension, were assessed in terms of depression, anxiety, stress and coping. RESULTS: The hypertensive group had higher scores of depression, stress and anxiety than the control one. Coping strategies were different between hypertensive and control groups (except for confrontive and self-reliant coping styles). When splitting up the hypertensive group into gestational hypertension, chronic hypertension and preeclampsia syndrome, differences between this new classification reached the statistical level. Our data suggests that women with preeclampsia have more symptoms of depression and worse coping strategies - they are less optimistic and more fatalistic. However, after a cluster analysis, two different subgroups of hypertensive women were found: one with worst coping strategies and more vulnerability to negative affective states and another with better coping and more resilient to mental health problems. LIMITATIONS: Data were cross-sectional. We excluded women with some comorbidities, such as a diagnosis of kidney disease, diabetes or fetal malformation. CONCLUSIONS: It is important to consider distinct profiles of pregnant women, in order to be able to better understand the peculiarities of mental health and coping during the gestation, especially in pregnancies complicated by hypertension.

OBJECTIVE: To analyze effects of the COVID-19 pandemic on the consumption of personal protective equipment and products (PPEP), as well as the frequency of surgical site infection (SSI) among non-COVID-19 patients submitted to cesarean sections. METHODS: A retrospective study was conducted in a maternity unity of a public teaching hospital which was not part of the reference service for COVID-19 treatment. It compared PPEP consumption and the occurrence of SSI after cesarean sections in monthly periods before and after the occurrence of the first case of COVID-19 in Porto Alegre, state of Rio Grande do Sul, Brazil. Personal protective equipment and products consumption was measured as units of masks, gloves, gowns, and caps, and use of alcohol-based products or soap for hand sanitation as ml/patient/day. The SSI index was calculated as the proportion of cases of SSI over the number of cesarean sections performed monthly during the study period. RESULTS: There was an increase in all measured items of PPEP, with consumption of disposable masks with a median of 1,450 units in the pre-COVID period, and of 2550 in the post-COVID period (a 75.9% increase). A decrease of 49% in SSI was detected, with a median of 1.74 in the pre-COVID period and of 0.89 in the post-COVID period. CONCLUSION: The increase in consumption of PPEP could be a result of safer practices adopted by healthcare workers with the advent of COVID-19, of which the following reduction in the occurrence of SSI could be a direct consequence. Despite the severity of the crisis, one could state that extreme situations can lead to valuable reflections and opportunities for improvement.

MéTODOS: Foi realizado um estudo retrospectivo em uma maternidade de um hospital público de ensino que não fazia parte do serviço de referência para o tratamento do COVID-19. Foram comparados o consumo de EPPI e a ocorrência de ISC após cesárea nos períodos mensais antes e após a ocorrência do primeiro caso de COVID-19 em Porto Alegre, RS, Brasil. O consumo de EPPI foi medido em unidades de máscaras, luvas, aventais e gorros, e o uso de produtos à base de álcool ou de sabonete para higienização das mãos em ml/paciente/dia. O índice SSI foi calculado como a proporção de casos de ISC sobre o número de cesarianas realizadas mensalmente durante o período do estudo. RESULTADOS: Houve aumento em todos os itens medidos do EPPI, com o consumo de máscaras descartáveis apresentando uma mediana de 1.450 no período pré-COVID e de 2550 no período pós-COVID (aumento de 75,9%). Detectou-se também diminuição de ISC, com medianas de 1,74 no período pré-COVID e de 0,89 no período pós-COVID, com redução de 49% no valor da mediana. CONCLUSãO: O aumento do consumo de EPPI pode ser resultado de práticas mais seguras adotadas pelos profissionais de saúde com o advento do COVID-19, do qual a redução na ocorrência de ISC pode ser uma consequência direta. Apesar da gravidade da crise, pode-se afirmar que situações extremas podem gerar reflexões valiosas e oportunidades de melhorias.

OBJECTIVE: The objective of this work is to investigate the role of personality in pregnancies complicated by hypertension, thru analysis of structure and associations between negative affect and coping strategies, and their role towards psychological distress. METHOD: A cross-sectional study with 343 women, where 192 pregnancies complicated by hypertension, was carried out by employing the following tools: the five-factor model (Big Five), Depression, Anxiety and Stress Scale (DASS-21), and Jalowiec's Coping Inventory. Two complementary strategies were carried out: an exploratory approach on the interactions between latent variables and a confirmatory technique. RESULTS: Coping strategies seem to be dissociated in the hypertensive group, and these participants tend to use strategies according to their personality, mostly emotion focused, extraversion, and neuroticism. The extraversion model exclusively shows an acceptable goodness-of-fit after a structural equation modelling. A multigroup analysis reached a full metric invariance level for extraversion. CONCLUSIONS: These results are of interest for both clinical and research settings. Prenatal screening and associated interventions may reduce perinatal negative affective states and related pregnancy complications.

OBJECTIVE: To analyze the prescription of antimicrobial agents for pregnant women admitted into the obstetrics service who presented with acute pyelonephritis. METHODS: Three cross-sectional studies were performed comparing the prescription of antimicrobials for pyelonephritis in pregnant women in the time periods evaluated (2010-2011: 99 patients evaluated; 2013: 116 patients evaluated; 2015: 107 patients evaluated), at the Hospital Fêmina, Porto Alegre, in the state of Rio Grande do Sul, Brazil. The analysis was performed before and after the promotion of an institutional protocol for the treatment of pyelonephritis during pregnancy, and on a third occasion after the introduction of a smartphone-based mobile educational tool. RESULTS: The evaluation of the prescribing physicians and the adequacy of the prescriptions between the different periods studied revealed a significant increase in appropriate conduct for the choice of antimicrobial (2010: 83.8%; 2013: 95.7%; and 2015: 100%), route of administration (2010: 97%; 2013: 100%; and 2015: 100%), and interval (2010: 91.9%; 2013: 95.7%; and 2015: 100%), following the introduction of the protocol, and again after the implementation of the software application with orientations on the antimicrobial treatment. CONCLUSION: The use of specific mobile applications should be encouraged to attain a better quality and accuracy in prescriptions and to include strategies that not only reduce the risk of negative outcomes, but also improve the quality of care and treatment for maintaining the health both of the mother and of the baby.

RESULTADOS: A avaliação das prescrições médicas e a adequação das prescrições entre os diferentes períodos estudados revelaram um aumento significativo na conduta adequada para a escolha do antimicrobiano (2010: 83,8%; 2013: 95,7%; e 2015: 100%), via de administração (2010: 97%; 2013: 100%; e 2015: 100%) e intervalo (2010: 91,9%; 2013: 95,7%; e 2015: 100%), após a introdução do protocolo, e novamente após a implementação do aplicativo com orientações sobre tratamento antimicrobiano. CONCLUSãO: O uso de aplicativos móveis específicos deve ser incentivado para obter melhor qualidade e precisão nas prescrições e incluir estratégias que não apenas reduzam o risco de resultados negativos, mas que também melhorem a qualidade dos cuidados e do tratamento para manter a saúde conjunta da mãe e do bebê.

AIMS: To validate the Brazilian version of the Fresno test of competence in Evidence-Based Medicine. METHODS: This is a cross-sectional, validation study. Phase 1: translation of the Fresno instrument. Phase 2: validation of the translated version, which was tested in 70 undergraduate medical students. The psychometric properties evaluated were validity, internal consistency, and sensitivity to change. RESULTS: Overall, validity was adequate; most items showed a moderate to strong and significant correlation with the total score; there was an important and significant difference between both groups, with and without previous contact with Evidence-Based Medicine (median, 55 [IQ25-75, 45.2-61.7] vs. median, 18.5 [IQ25-75, 6.0-29.7]) (p<0.001). Internal consistency was also adequate (α-C 0.718), and sensitivity to change showed a considerable and significant difference between pre and post-test (median, 18.5 [IQ25-75, 6.0-29.7] vs. median, 44 [IQ25-75, 34.0-60.0]) (p<0.001). CONCLUSIONS: The Brazilian version of the Fresno test showed satisfactory psychometric properties, and it can now be used as a tool to assess the knowledge and skills of Evidence-Based Medicine in Brazilian medical students.


OBJETIVOS: Validar a versão brasileira do teste Fresno de competência em Medicina Baseada em Evidências. MÉTODOS: Este é um estudo transversal de validação. Fase 1: tradução do instrumento Fresno. Fase 2: validação da versão traduzida, testada em 70 estudantes de graduação em medicina. As propriedades psicométricas avaliadas foram validade, consistência interna e sensibilidade à mudança. RESULTADOS: No geral, a validade foi adequada; a maioria dos itens apresentou correlação moderada a forte e significativa com o escore total; houve diferença importante e significativa entre os dois grupos, com e sem contato prévio com medicina baseada em evidências (mediana, 55 [IQ25-75, 45,2-61,7] vs. mediana, 18,5 [IQ25-75, 6,0-29,7]) (p<0,001). A consistência interna também foi adequada (α-C 0,718), e a sensibilidade à mudança mostrou uma diferença considerável e significativa entre o pré e o pós-teste (mediana, 18,5 [IQ25-75, 6,0-29,7] vs. mediana, 44 [IQ25-75], 34,0-60,0]) (p<0,001). CONCLUSÕES: A versão brasileira do teste Fresno mostrou propriedades psicométricas satisfatórias, e agora pode ser usada como uma ferramenta para avaliar o conhecimento e as habilidades da Medicina Baseada em Evidências em estudantes de medicina brasileiros.

ABSTRACT Learning is a complex construct that involves several factors, mainly the interaction between teachers and students in the process of teaching and learning. Understanding how students learn and which factors influence academic performance is essential information for lesson planning and evaluation, in addition to allowing a better use of students' learning potential and outcomes. The ability to constructively modify one's behavior depends on how well we combine our experiences, reflections, conceptualizations, and planning to make improvements. This seems particularly relevant in medical education, where students are expected to retain, recall, and apply vast amounts of information assimilated throughout their training period. Over the years, there has being a gradual shift in medical education from a passive learning approach to an active learning approach. To support the learning environment, educators need to be aware of the different learning styles of their students to effectively tailor instructional strategies and methods to cater to students' learning needs. However, the space for reflection on the process of teaching is still incipient in higher-education institutions in Brazil. The present article proposes a critical review of the importance of identifying students' learning styles in undergraduate medical education. Different models exist for assessing learning styles. Different styles can coexist in equilibrium (multimodal style) or predominate (unimodal style) in the same individual. Assessing students' learning styles can be a useful tool in education, once it is possible to analyze with what kind of learning students can better develop themselves, improving their knowledge and influencing positively in the process of learning. Over the last century, medical education experienced challenges to improve the learning process and curricular reform. Also, this has resulted in crucial changes in the field of medical education, with a shift from a teacher centered and subject based teaching to the use of interactive, problem based, student centered learning.


RESUMO A aprendizagem é uma construção complexa que envolve diversos fatores, principalmente a interação entre professores e alunos no processo de ensino/ aprendizagem. Entender como os alunos aprendem e quais fatores influenciam o desempenho acadêmico são informações essenciais para o planejamento das aulas, além de permitir um melhor aproveitamento do potencial de aprendizado e desempenho dos alunos. A capacidade de modificar construtivamente o comportamento de uma pessoa depende de quão bem combinamos nossas experiências, reflexões, conceituações e estratégias para desenvolver o processo de mudança. Isso parece particularmente relevante na educação médica, na qual se espera que os alunos retenham, processem e apliquem grandes quantidades de informação durante todo o período de treinamento. Ao longo dos anos tem havido uma mudança gradual na educação médica de uma abordagem de aprendizagem passiva para uma abordagem de aprendizagem ativa. Para fortalecer o ambiente de aprendizado, os educadores precisam estar cientes dos diferentes estilos de aprendizado de seus alunos e, desta forma, adaptar estratégias e metodologias pedagógicas que aprimoram o processo de aprendizagem. No entanto, o espaço de reflexão sobre o processo de ensino ainda é incipiente nas instituições de ensino superior no Brasil. O presente artigo propõe uma revisão crítica sobre a importância da identificação dos estilos de aprendizagem dos alunos no ensino médico de graduação. Existem diferentes ferramentas para avaliar estilos de aprendizagem. Diferentes estilos podem coexistir em equilíbrio (estilo multimodal) ou predominar (estilo unimodal) no mesmo indivíduo. Avaliar os estilos de aprendizagem dos alunos pode ser uma ferramenta útil na educação, uma vez que é possível analisar as vias sensoriais mais favoráveis para assimilar e processar os conhecimentos, influenciando positivamente o processo de aprendizagem. No último século, a educação médica vem postulando novos desafios para melhorar o processo de aprendizagem através da reforma curricular. Além disso, impulsionou mudanças cruciais no campo da educação médica, transformando um modelo de ensino passivo, previsível e centrado na figura do professor em um modelo de aprendizagem ativo, centrado no aluno, interativo e baseado em problemas.

BACKGROUND: The relationship between personality and health is frequently studied in scientific research. This study investigated the clinical/biochemical course of kidney transplant patients based on personality traits. METHODS: A longitudinal study assessed 114 kidney transplant patients (men = 68 and women = 46) with an average age of 47.72 years (SD = 11.4). Personality was evaluated using the Brazilian Factorial Personality Inventory (BFP/Big Five Model). Clinical variables were analyzed based on patient charts (estimated glomerular filtration rate (eGFR), hypertension, acute rejection, infection, graft loss, and death). Personality types were assessed by hierarchical cluster analysis. RESULTS: Two groups with personality types were differentiated by psychological characteristics: Cluster 1 - average neuroticism, high surgency, agreeableness and conscientiousness, and low openness; Cluster 2 - high neuroticism, average surgency and agreeableness, average conscientiousness, and low openness. There was no statistically significant difference between the clusters in terms of hypertension, acute infection, graft loss, death, and Human Leukocyte Antigen (HLA) I and II panel reactive antibodies. eGFR was associated with the personality types. Cluster 2 was associated with a better renal function in the 9-month follow-up period after kidney transplantation. CONCLUSION: In this study, patients from Cluster 2 exhibited higher eGFR 9 months after the transplant procedure compared to those from Cluster 1. Monitoring these patients over a longer period may provide a better understanding of the relationship between personality traits and clinical course during the post-transplant period.

Reflexões sobre o voluntariado como atividade de estudantes de medicina, apresentadas por Sapiro e Mattiello nesta edição da Scientia Medica, trazem à tona o pensamento de que quem beneficia tem que entender porque beneficia, que é o que ocorre com adultos. E para o entendimento do que seja o altruísmo, tema familiar à medicina através dos tempos, não basta a leitura do que já foi produzido sobre o assunto, mas é inevitável a exposição à prática do benefício. Evidências observacionais sugerem que o voluntariado pode beneficiar a saúde mental e a sobrevida dos indivíduos expostos, embora os mecanismos causais permaneçam desconhecidos. Consequentemente, estudos futuros são necessários e devem esclarecer mais sobre o assunto e enquanto isto não acontece pode-se pensar sobre uma citação: "If you would like to be selfish, you should do it in a very intelligent way. The stupid way to be selfish is … seeking happiness for ourselves alone. … the intelligent way to be selfish is to work for the welfare of others." The Dalai Lama.


The contemplation of volunteerism as activity in medical school, put forward by Sapiro and Mattiello in the present issue of Scientia Medica, brings to light the idea that those who provide benefits have to understand why they do so, as occurs among adults. And to understand selflessness, a topic that has been recurrent in medicine over time, reading about what has already been produced is not enough, since exposure to the practice of concern for the welfare of others is essential. Observational evidence suggests that volunteerism may be beneficial to the mental health and survival of exposed individuals, even though the causal mechanisms remain unknown. Consequently, further studies are needed to shed more light upon the topic, but while that does not occur, the following quotation gives us some food for thought: "If you would like to be selfish, you should do it in a very intelligent way. The stupid way to be selfish is … seeking happiness for ourselves alone. … the intelligent way to be selfish is to work for the welfare of others." The Dalai Lama.

ABSTRACT Background - The increase in the survival following the introduction of highly active antiretroviral therapy (HAART) has seen the emergence of hepatitis C virus (HCV) infection, renal and cardiovascular diseases as important morbidity and mortality causes together with HIV. Objective - The present study aimed to investigate the differences between HIV/hepatitis C virus coinfected and HIV-monoinfected regarding demographic and clinical aspects from a HIV/AIDS clinic in Porto Alegre, Brazil. Methods - Review of medical records of 1,030 HIV infected individuals aged 18 years or more in an urban HIV/AIDS clinic based in Porto Alegre, Southern Brazil. Clinical and demographical Data were collected from the records of the patients attended between March 2008 and December 2012. Results - The present study is a cross-sectional study among HIV-infected patients attended at a public HIV/AIDS clinic in Porto Alegre, Brazil. The prevalence of hepatitis C virus in the present study cohort was 11.8% (CI 95%: 9.9%-13.8%). Hypertension and pathological proteinuria were more common in the coinfected compared to monoinfected group. By the other hand, dyslipidemia were more common among monoinfected patients. There was no difference between the groups regarding CD4+ count or HIV-RNA. Variables significant in the univariate analysis with P<0.05 were further analyzed using a Poisson regression model with robust variance. Coinfected were likely to be older, with lower lipid levels and higher prevalence of pathological proteinuria compared to HIV-monoinfected patients. Although coinfected patients had higher prevalence of tenofovir-based regimen, there was a strong association between hepatitis C virus individuals to pathological proteinuria and dyslipidemia. Conclusion - Clinicians should recognize that coinfected and monoinfected individuals are different groups regarding the traditional and HIV-related risk factors and should be managed and screened individually in order to prevent cardiovascular and renal complications.


RESUMO Contexto - O aumento da sobrevida após a introdução da terapia antirretroviral nos pacientes vivendo com HIV tem como consequência o aparecimento de doenças emergentes nestes pacientes, como a hepatite pelo vírus C, doenças renais e cardiovasculares. Objetivo - O presente estudo tem como objetivo investigar as diferenças entre monoinfectados por HIV e coinfectados por HIV/virus da hepatite C, considerando aspectos demográficos e clínicos de pacientes atendidos em uma clínica de HIV/AIDS em Porto Alegre, Brasil. Métodos - Revisão de prontuários médicos de 1.030 indivíduos vivendo com HIV em uma clínica especializada em Porto Alegre, Brasil. Dados clínicos e demográficos foram coletados a partir dos registros dos pacientes atendidos entre março de 2008 e dezembro de 2012 na referida clínica. Resultados - O presente estudo é um estudo transversal com indivíduos vivendo com HIV, atendidos em um serviço municipal de HIV/AIDS em Porto Alegre, Brasil. A prevalência de hepatite pelo vírus C na presente coorte de estudo foi 11,8% (IC 95%: 9,9%-13,8%). Hipertensão e proteinúria patológica eram ocorrências mais comuns em coinfectados do que monoinfectados. Por outro lado, dislipidemia foi mais comuns entre monoinfectados. Não houve diferença entre os grupos quanto contagem de linfócitos CD4 totais ou HIV-RNA. Variáveis significativas na análise univariada com P<0,05 foram ainda analisadas usando um modelo de regressão de Poisson com variância robusta. Coinfectados eram mais velhos, com os níveis de lipídios mais baixos e maior prevalência de proteinúria patológica em comparação com indivíduos monoinfectados. Apesar de os coinfectados apresentarem maior prevalência de estarem em uso de regime contendo tenofovir, houve uma forte associação dos indivíduos infectados pelo vírus da hepatite C com proteinúria patológica e ausência de dislipidemia. Conclusão - Clínicos devem reconhecer que coinfectados e monoinfectados pertencem a grupos diferentes quanto aos fatores de risco tradicionais e aqueles associados ao HIV, devendo estes serem manejados e rastreados de forma individual, para prevenir complicações cardiovasculares e renais.

Lesão renal aguda (LRA) é definida como a perda rápida da função renal de filtração e está presente em diversos pacientes que procuram atendimento em unidades de emergência. As diferentes etiologias e os níveis variados de gravidade representam uma situação desafiadora ao médico emergencista, que deve ter domínio intimidade com o manejo inicial da patologia.


Acute kidney injury (AKI) is defined as an rapid decline in renal filtration function and is often present in patients that seek the emergency department. The different etiologies and multiple classifications of severity represent a challenging situation to the emergencist that must have thorough knowledge of the initial approach to the disease.

OBJECTIVES: HIV-related renal diseases are the leading causes of chronic kidney diseases worldwide. The present study aimed to investigate the prevalence of pathological proteinuria and its risk factors among HIV patients. METHODS: A review of the medical records of 666 HIV-infected individuals aged 18 years or older in an urban HIV/AIDS clinic based in Porto Alegre in southern Brazil. Overt proteinuria was defined as a protein-to-creatinine ratio greater than 150 mg/g according to Kidney Disease: Improving Global Outcomes. RESULTS: The prevalence of pathological proteinuria in the present study cohort was 20%. Characteristics associated with pathological proteinuria after univariate analysis included alcohol abuse, hepatitis C virus coinfection, the occurrence of diabetes and therapy including tenofovir. Adjusted residuals analysis indicated an association between pathological proteinuria and both a CD4 lymphocyte count below 200 cells/mm3 and a viral load higher than 1000 copies/mL. Additionally, an absence of pathological proteinuria was associated with a CD4 lymphocyte count higher than 500 cells/mm3. After adjustment for variables with p<0.2 in the univariate analysis using a Poisson regression model, tenofovir-containing regimens and a CD4 lymphocyte count below 200 cells/mm3 were significantly associated with pathological proteinuria. CONCLUSION: The risk of chronic kidney diseases in this large contemporary cohort of HIV-infected individuals appeared to be attributable to a combination of HIV-related risk factors. In addition to the traditional risk factors cited in the literature, both regimens containing tenofovir and HIV disease severity seem to be associated with chronic kidney diseases in patients with HIV. Assessment of proteinuria constitutes a novel method for chronic kidney disease staging in HIV-infected individuals and may be effectively used to stratify the risk of progression to end-stage renal disease.

Patients with HIV may have an increased risk of hypertension and cardiovascular disease (CVD). The objective of this study was to determine the prevalence and risk factors for hypertension in a population of HIV-infected patients at an HIV/AIDS clinic in southern Brazil. We reviewed medical records of 1009 HIV-infected patients aged 18 years or more in an urban HIV/AIDS clinic based in Porto Alegre, southern Brazil. Hypertension was defined according to the Eighth Joint National Committee criteria. The prevalence of hypertension in this study cohort was 22.5% (95% confidence interval, 20%-25.2%). Individuals were significantly older in the hypertensive group (P < .001). After adjustment using a Poisson regression model of all variables that presented P < .2 in the univariate analysis, the variables that were significantly associated with hypertension were only age ≥40 years and obesity. Also in this setting, dyslipidemia (P = .068) showed a tendency of association with hypertension. Compared with HIV-infected persons aged 18-39 years, those aged 40-59 years presented a 2-fold higher prevalence of hypertension (95% confidence interval, 1.2-3.3).The present study showed a high prevalence of hypertension among HIV-infected persons, similar to other studies, ranging from 13% to 45%, and also similar to the HIV-negative general population. Age and obesity were the factors associated with hypertension. Finally, the present study indicates a similar pattern of behavior and comorbidities for HIV-positive and -negative patients in relation to hypertension.

To determine the correlation between protein-to-creatinine ratio and 24-h urinary protein, proteinuria was measured in 45 patients attending a public HIV clinic in Porto Alegre, Brazil, using 24-h urinary protein excretion (24hUP) and urinary protein-to-creatinine ratio. Spearman's correlation test was done to evaluate the association between spot protein-to-creatinine ratio and 24hUP. The limits of agreement between the two methods were analysed by the Bland-Altman method. For protein excretion <1 g/day, limits (95%) of agreement of protein-to-creatinine ratio and 24hUP were +0.112 and -0.097 g/day. A strong correlation (r = 0.957) was found between protein-to-creatinine ratio and 24hUP excretion. The conclusion is that the protein-to-creatinine ratio in spot urine specimens is an accurate, convenient and reliable screening method to estimate the urinary protein excretion in HIV patients to detect abnormal urinary protein loss. Further studies are required to evaluate renal disease in HIV patients with chronic renal disease and higher urinary protein excretion.

Ramsay Hunt Syndrome (RHS) is the result of herpes zoster virus reactivation producing hearing loss, pain and vesicles in the ear or mouth, along with ipsilateral facial palsy due to the 7th cranial nerve geniculate ganglion infectious involvement. This condition has not been previously described, particularly in transplant patients. A 38-year old man underwent kidney transplantation and two years later experienced an ache on the left side of the face and hearing loss in the ear, also exhibiting vesicular lesions and concomitant facial peripheral palsy. Acyclovir IV was initiated, and the prednisone dose was increased. The patient was discharged 15 days later, feeling better but still exhibiting dark spots on his face. At three months follow-up he was asymptomatic, showing notable palsy improvement. Until this case, herpes zoster facial lesions causing typical RHS have never been reported in literature, particularly in kidney transplant patients.
